Player Participation Confirmation

My player will be participating in the Bradford Minor Hockey Girls Club program for the 2025-26 season. She is currently registered and insured with South Simcoe Minor Hockey through the OMHA.

She will be taking part in Bradford Girls Club events and activities at the Bradford Leisure Centre throughout the season.
